Paperwork that Might Be Needed

Generally, once you are registered for SummerKids, you won't need to send more paperwork.  However, in some circumstances you will need more:
  •  Activity Transportation Scheduling:  If you sign your child up for community activities and you want us to get your child there, you will need to request that service.  Please see "Activity Transportation Scheduling" link to the left.
  • Medication Form - If your child will need to be given any medication during our program hours, including over the counter, on a regular or emergency basis, you must complete a Medication Form. Contact us as soon as possible if you need this form.
  • Emergency Response Plan. If your child has any condition that may be life threatening or require special care, please provide us with a copy of the Emergency Response Plan that is filed with the school. Examples include, but are not limited to, life threatening allergies, seizure disorders and diabetes
  • Additional Information about your Child - Please complete this form if your child has any special needs or behaviors that we may need help in understanding. This will allow us to serve your child in the best possible way.
